Hi Cameron,
This error could indicate that there's an issue with the UGC definition, most likely duplicate column names. Check the UGC definition for any duplicates.
If you are stuck on this, we may need to intervene to resolve. Please contact support@agilitycms.com and provide the Agility Instance name and the name of the UGC definition.
